Observation

Parent filled out a medication management form on May 14, 2021. The form instructs that if the child would be outside for any time greater than 15 min that sunscreen needed to be applied. Communication between the staff broke down leading to the staff in the room not knowing that the parent had filled out and provided sunscreen. The child was taken outside for longer than 15 minutes without the provider applying sunscreen. This resulted in the child recieving a sunburn.
